AXXESS Registration

AXXESS registration is open to all LMHSC, Inc., enrollees. 

· If you are not a current LMHSC, Inc., enrollee, an application and enrollment fee must be submitted with the registration form

· Class fees are $30 per class. An administrative fee of $40 is assessed to each family per hour per year.

· Administrative fees will be waived if parent volunteers during the time their children are involved in classes.

· Volunteers will be assigned tasks.  Every effort will be made to accommodate your needs.  Administrative fees will be waived for staying and being available for volunteer service

Axxess Fee Payment Schedule

     Þ  One-third of all fees deposit due with registration.  Class     fees deposit is non-refundable. 

     Þ  One-third payment first day of first semester, September 12, 2008

     Þ  One-third payment first day of second semester, January 9, 2009

· A $10 late fee will be assessed on  fees not paid when due

· Your LMHSC account must be current to register for new classes.

· A scholarship fund is available for session fees for those with expressed financial need.

· You may only register your own custodial children.

· If no class is desired please indicate study hall.

· Class lists and task assignments will be posted at www.lmhsc.org/data after close of registration.

Axxess Registration Guidelines

A non-refundable deposit of one-third of fees required for all registrations.

Registrations are taken on a first-come, first-served basis.

Late registrations will require a $10 late fee.

Many classes will close after registration deadline.  Tutor discretion & space will determine availability.

Registrants must also enroll in LMHSC, Inc.  (Be sure to complete an enrollment & session registration form)
