Muskegon CO-OP Registration Requirements

  • CO-OP registration is open to all LMHSC, Inc., enrollees.  At least one child must be enrolled in the kindergarten through 12th grade classes.
  • If you are not a current LMHSC, Inc., enrollee, an application and enrollment fee must be submitted with this form.
  • Fees are $40 per session ($24 per session for families with only ONE child at CO-OP).  An administrative fee of $80 is accessed to each class per child per session. Administrative fees will be waived if parent volunteers during the time their children are involved in classes.  Material and administrative fees due on 1st day of classes
  • Volunteers will be assigned tasks.  Every effort will be made to accommodate your needs.  Tasks will only be changed if you are physically unable to do the assigned task.
  • A $10 deposit is required with all registrations.  Material fees & administrative fees are due the first day of classes.
  • A $10 late fee will be assessed on ALL fees not paid on first day of classes.
  • Your LMHSC account must be current to register for new classes.
  • A scholarship fund is available for session fees for those with expressed financial need.
  • Please list all children who will attend CO-OP with you.
  • You may only register your custodial children.
  • Please indicate first and second choice of class for each child.
  • If first choice is filled, your child will automatically be placed in the second choice.
  • If no class is desired please indicate game room or study hall.
  • Choose your child’s classes carefully.  After registration is closed, you will not be able to move your child to another class.  You may drop a child from a class; however, they must then be enrolled in either game room or study hall THEREAFTER.
  • Class lists and task assignments will be posted at www.lmhsc.org/data after close of registration.

$10 non-refundable deposit required for all registrations.

Registrations are taken on a first come, first served basis.

Late registrations will require a $10 late fee.

Many classes will close after registration deadline. Time allowance, tutor discretion & space will determine availability.

Registrants must also enroll in of LMHSC, Inc. (Be sure to complete an enrollment & session registration form. Please allow two weeks for enrollment application processing.)

We have read the policies for LMHSC, Inc., and agree that our family will abide by them. We have read the notice at the beginning of this form and understand all items. We understand that if we have signed our children up for a P.E./Gym class, there is always a possibility of physical injury, and we accept responsibility for that decision. We will contact LMHSC, Inc., as soon as possible if we are unable to fulfill our commitment to attend classes, so that other students, who may be on a waiting list, can take our child’s place. We understand that late cancellations (one week or less prior to the beginning of classes) require a 50% payment of the session fee PLUS any materials and administrative fees. We understand that fees are still owed if family is absent. We understand that in order to register to subsequent sessions at CO-OP our account must be current. We also understand that all fees must be paid in full by the last week of the session. We understand that if we leave our child at the CO-OP facility at any time, that we must have another adult responsible for our child in the building and complete a corresponding permission slip/insurance authorization.

**NOTE**I understand that failure to complete, being late (10 minutes or more), chronic tardiness (two times or more of being 5 minutes or more late), or not showing for said tasks may result in administrative fees being assessed for the hours missed. Fees will be prorated in proportion to the total session administrative fee ($10 per hour per student). I understand that if I am absent due to illness that this does not apply. In the case of an absence due to illness, I will call as promptly as possible to let the committee know. However, if I plan to miss any weeks of the session, I have already notified committee and have made arrangements for my task assignments. Music

Our band and orchestra programs offer beginning and intermediate classes. We have a band program with a full range of instruments: flute, clarinet, trumpet, trombone, oboe, saxophone and percussion. We also have an orchestra program introducing students to the world of strings through violin, viola, cello and guitar. Read more…
